I have recently ran into a couple situations where I wanted to remove a Firefox 3 extension only to find that the Add-On management screen would not let me uninstall the add-on. The uninstall button was grayed out. After doing some searching I figured out how to remove it. Delete the registry key that tells Firefox to load the add-on.
